To apply to Camp Holiday Trails, CONTACT US or download, print, complete, and return the forms above! To ensure we can provide the best possible care for our Campers, applications cannot be reviewed until all forms are returned.

Which Forms Do I Have to Fill Out?

If a campers is attending Session 1, 2, 3, or 4 you need to fill out the Camper Application

(including all Financial Aid Forms).

If the Camper will also be 15-17 years old at camp this summer he/she will be a Blazer! A new and updated Blazer Teen Leader Program is in progress and ALL campers ages 15-17 attending Sessions 1-3 is required to fill out the Blazer Input Survey!

If the Camper is a sibling attending Session 1 (Young Camper Week) or Session 4 (Camp Youngblood)

you need to fill out the Sibling Application.

If your Family will be attending Family Camp Week

or Family Hemophilia Camp Weekend or

another Family Camp you will need to fill out

the Family Camp Application.

work hard year-round to raise the funds needed toensure

every child can attend Camp, regardless of ability to pay.

We understand that attending Camp requires and

investment of time and money - and we encourage

families to partner with our Camp in raising the needed funds.
